What personal information do we collect and why?

We may collect personal information about you (such as your first and last name, mailing address, telephone number, e-mail address, your next of kin), directly from you, when you use the Website, Needs Assessors (NASC) Referral, over the phone, by letter or email.

We will only use and disclose your personal information for the purposes for which it was collected (or otherwise as required or authorised by law). This may include using your personal information for the following purposes:

(a) To provide you with services and to personalise your experience when browsing the Website.
(b) To undertake customer service activities, such as responding to any queries, comments, or complaints you may have.

If you do not want your personal information to be collected, please do not submit it to us. Please note that if you elect not to provide certain personal information to us, we may not be able to provide you with the services, products, or information you require.
