Long-Term Chronic Health Care Services

Long-term chronic health care is for people of all ages with physical, sensory and/or intellectual impairments who are, or want to live independently in their community. Pacific Homecare offers the following long-term chronic health care services:

Personal Care

Personal care includes assistance with getting you ready to face the start and the end of each day. For example, showering, bathing, washing and drying hair, shaving, dressing and undressing, toileting, mobility, meal preparation and feeding

Household Management

Household management includes assistance with everyday household tasks that you may be struggling to complete. For example, cleaning, laundry, ironing, vacuuming or mopping the floors, doing the dishes, dusting

Social Support

Social support allows people with disabilities to remain active in the community and encourages them to participate as fully as they are able. Social support involves the following:

Mental Support

Mental support including emotional empathy, companionship and encouragement in meeting your personal goals Information sharing.This involves putting you in touch with other agencies and groups to increase your interaction and participation in the community.

For example, suppliers of hearing aids, wheelchairs, manual hoists and other clinical equipment; workshops, community activities and forums where you have fun developing your full potential.

Physical support

This includes transportation to the shops, events, church and other social activities; shopping, assisting with homework, reading, writing and musical activities.

Complex Long-Term Care

Complex long-term care is assistance for people with serious health conditions or injuries, many of which may leave them bed-bound or in a wheelchair. We provide assistance in dealing with cerebral palsy, Muscular Dystrophy, intellectual disability, Downs syndrome, Multiple Sclerosis, epilepsy, autism, etc

I call my Support Workers ‘my girls’ because they started off as workers, then became friends, and now they’re just like family. With them, I know my life is complete. I don’t have to worry about who is going to be here or worry that I need things done for me or the house. I can rely on them.

Steps for Service

Doctor's Referral

  • Step 1

GP referral to NASC/Taikura (customer goes to their GP to send in-home referral request to NASC/Taikura)

Your Referral

  • Step 2

Your referral is sent to Pacific Home Care from DHB/Taikura NASC.

In-Home Visit

  • Step 3

Our Clinical Coordinator will visit with family and together we can agree to a Care Plan.

Support Worker Placement

  • Step 4

Our Resource Coordinator will match the most suitable Support Worker to you 

Schedules & Rosters

  • Step 5

The roster is created based on your care plan and then service will begin.
