Individualised Funding

Individualised Funding (IF) is a way of paying for elderly and disability support services. IF enables elderly and disabled people to directly manage the hours they are allocated for Home and Community Support Services.

Pacific Homecare works alongside clients, family and whanau to support and plan how best to use the hours of support by employing care workers and managing all aspects of the service delivery.

Services are flexible, responsive and needs based. Pacific Homecare offers the following Individualised Funding services:

PERSONAL CARE includes assistance with getting you ready to face the start and the end of each day. For example, showering, bathing, washing and drying hair, shaving, dressing and undressing, toileting, mobility, meal preparation and feeding

HOUSEHOLD MANAGEMENT includes assistance with everyday household tasks that you may be struggling to complete. For example, cleaning, laundry, ironing, vacuuming or mopping the floors, doing the dishes, dusting

SOCIAL SUPPORT allows people with disabilities to remain active in the community and encourages them to participate as fully as they are able by creating linkages that allow their needs to be addressed holistically in an environment most appropriate to them.

Social support involves the following:

COMPLEX  CARE is assistance for people with serious health conditions or injuries, many of which may leave them bed-bound or in a wheelchair. We provide assistance in dealing with cerebral palsy, Muscular Dystrophy, intellectual disability, Downs syndrome, Multiple Sclerosis, epilepsy, autism, etc

These services focus on the client with an impairment and where relevant, their family and whanua, to enable people to make real decisions about their own lives.
